OK, I'll answer my own question. It came down to doing a bit of
research and finding the right resource.

In the end, the best recipe for getting ldap authentication to work was
on the Debian Wiki:

http://wiki.debian.net/LdapAuthentication

There are a few minor errors in the documentation (perhaps it is because
things have changed in the packages since woody came out) but with a
small amont of fiddling, I was able to get everything up and running.
Phew! That was 4 days of work just to get to the right answer which
took 15 minutes to implement.

Hopefully this post will serve as a signpost for others who have the
same problem.
